Cognizant is excited to be launching the Wellbeing Program to all full-time U.S. Associates! Login to your myHealthCheck360 account to start earning points by completing wellbeing activities. New this year, those who achieve 10,000+ points will be entered into raffle prize drawings for credits to the Rewards Mall. Here you will have access to a robust catalog of prizes including electronics, appliances, gift cards, travel and more!

 

September: 50 lucky associates will be drawn to win 600 credits
October: 50 lucky associates will be drawn to win 400 credits
November: 50 lucky associates will be drawn to win 200 credits
December: 50 lucky associates will be drawn to win 100 credits
(1 credit is equivalent to ~$1 USD)

Go to myHealthCheck360 and use Company Code CTS25. Your Unique ID will vary based on your employment status:
 
 Cognizant Associates: full associate ID (i.e.123456)
Collaborative Solutions Group Associates: last 4 of associate ID (i.e.1234 if full ID is 001234)
Spouses/Domestic Partners: same unique ID as the associate, but add a "1" to the end (i.e. using examples above, unique ID would be 1234561 or 12341)
