How do I create an account?

Associates can take the steps below to create a HC360 account. Please note: All members are required to create their own personal account to participate. 

 

  1. Download the myHealthCheck360 mobile app from the Apple App Store or Google Play Store
  2. Click Create an Account
  3. Enter your personal details
    • Company Code: CTS25
    • Enter your email address
    • Unique ID: Varies depending on employment status
      • Cognizant Associates: full associate ID (i.e. 123456)
      • Collaborative Solutions Group Associates: last 4 of associate ID (i.e. 1234 if full ID is 001234)
  1. Create a username and password that meet the requirements
  2. Check the box next to Terms and Conditions. 

You can also create an account or log in at www.myhealthcheck360.com.

 

ALL MEMBERS ARE REQUIRED TO CREATE THEIR OWN, PERSONAL ACCOUNT TO PARTICIPATE.

What is my company code?

CTS25

Are spouses and domestic partners eligible to participate?

If you are the spouse/domestic partner of a US Associate who is on the health plan or is planning to join the health plan in the upcoming year, you are able to create an account and utilize the resources within the myHealthCheck360 platform (trackers, challenge your friends, goal setting, etc.).  To create your account, follow the steps above. Your unique ID is the same as the associate, but add a "1" to the end (i.e. using examples above, unique ID would be 1234561 or 12341); connect with associate if you are unsure of their ID.

Spouses/Domestic Partners are not eligible for the Lifestyle Rewards program to be eligible for the raffle prize drawings awarded during the program year. You will be eligible for the Health Plan Incentive Program*. 

* Complete requirements to earn discounted medical plan premiums starting in January 2025. This program is not currently available. You will receive more information from both Cognizant and HealthCheck360 on this program soon.

I'm a new associate. Can I still participate?

  • I am a full-time US Associate: All Full-time US Associates have access to create an account and participate in the Cognizant Wellbeing Program. Those who participate in the wellbeing-related activities and achieve the 10,000-point goal will be eligible for prize drawings starting in Quarter 3. You are also eligible to participate in the Health Plan Incentive Program* that allows you the opportunity to earn lower premium rates starting January 1 of the following year.
  • I am a full-time Canadian Associate: All Full-time Canadian Associates have access to create an account and utilize the resources within the myHealthCheck360 platform (trackers, challenge your friends, goal setting, etc.). You are also eligible to participate in the Cognizant Global Wellbeing challenges offered throughout the year for your chance to compete against your colleagues. Canadian Associates are not eligible to participate in either the Lifestyle Rewards Program OR the Health Plan Incentive Program.

* The Health Plan Incentive Program is not currently available to US Associates and their spouses/domestic partners. Cognizant and HealthCheck360 will be sending more information via email and mailers as we approach the launch of this program in Quarter 2.

How do I submit for Lifestyle Rewards Activities to earn points?

On the myHealthCheck360 App

  1. At the bottom select Rewards. 
  2. Select + Add.
  3. Select your activity from the drop down menu.
  4. Type a brief activity description, enter a date, and upload your document if required.
    • Certain activities do require you to upload some form of documentation. Be sure to check out the description when choosing your activity from the list to understand what is required from you, the associate.
  5. Select + Add to submit your activity.

 

On Your Computer at www.myhealthcheck360.com

  1. Click Lifestyle Rewards on the left hand menu.
  2. Click the blue Submit Activity box.
  3. Select your activity from the drop down menu.
  4. Type a brief activity description, enter a date, and upload your document if required.
    • Certain activities do require you to upload some form of documentation. Be sure to check out the description when choosing your activity from the list to understand what is required from you, the associate.
  5. Select Submit to submit your activity.

For activities that require a file to be uploaded, the limit is 10 MB. Accepted file formats include: .pdf, .jpg, .png, .gif, .doc, .docx

What can I earn by participating in the Lifestyle Rewards Program?

US Associates who participate in the Lifestyle Rewards program and earn at least 10,000 points will be entered into monthly drawings beginning in September. Winners will be awarded credits to the NEW Rewards Mall where you will have access to choose from a variety of items including electronics, workout equipment, gift cards, travel vouchers, and so much more!

Winners will be drawn monthly starting at the beginning of September and running through December. Winners will be required to collect their winnings and utilize their credits by 12/31/2024.

September: 50 lucky associates will be drawn to win 600 credits
October: 50 lucky associates will be drawn to win 400 credits
November: 50 lucky associates will be drawn to win 200 credits
December: 50 lucky associates will be drawn to win 100 credits


(1 credit is equivalent to ~$1 USD - taxes and shipping costs will be factored into the prize value prior to checking out)

How do I know if I am selected as a winner for the Rewards Mall credit?

If you are selected as a winner, you will receive an email from CognizantWellbeing@healthcheck360.com notifying you that you have won and you will be asked to accept/deny the prize. If you choose to accept, you receive access to your Rewards Mall credit within 10-business days along with instructions on how you can use your credits.

How do I connect a device or app?

Mobile App Instructions:

  1. Log in to the myHealthCheck360 app
  2. Select the Menu Icon (≡) in the upper left hand corner
  3. Select Connect Apps and Devices
  4. Find your fitness device or app and select connect
  5. Follow the prompts to log into your app or device's account and follow the instructions. If prompted, select all data to be shared. This helps the data pull over correctly
  6. You will get a confirmation that your device is connected.

Online Portal Instructions:

  1. Log in to myHealthCheck360.
  2. Select your username icon in the top right corner of your dashboard
  3. Select Connect a Device/App
  4. Find your fitness device or app and select connect
  5. Follow the prompts to log into your app or device's account and follow the instructions. If prompted, select all data to be shared. This helps the data pull over correctly
  6. You will get a confirmation that your device is connected
  7. Note that Apple Health can only be connected from the myHealthCheck360 mobile app

How do I know if my device is supported?

View the complete list of supported devices here. If you do not wish to utilize one of these apps, please use the trackers located in the myHealthCheck360 app or portal to manually track your health.

Will my information be confidential?

Yes! HealthCheck360 follows the same confidentiality standards as your doctor. Protected health information at an individual level will not be shared with any other parties, including Cognizant. To review our Privacy Policy, please click here.

Who should I contact with questions regarding the Cognizant Wellbeing Program?

Who should I contact with questions regarding my health plan coverage or to locate a primary care physician?

If you have medical coverage through a Cognizant Health Plan contact Included Health at 844-252-3056 for help understanding your coverage or for assistance in finding a physician. If you do not have benefits through Cognizant and have questions regarding your coverage, please direct these to your specific insurance provider.
