Create Your Account 

  • Download the myHC360+ mobile app or visit https://my.hc360.app/auth/sign-in.
  • Select Register and enter your information. If using the website, select "Start your journey!"
  • Enter your Company code. (Your company code is a 5-character code, unique to your employer, used to connect you with your wellness program. The code is included in the registration instructions provided by your company or HealthCheck360.) 
  • Select "Begin Registration
  • Your unique identifier is your last 4 SSN
  • Ensure you are entering a valid email. You will need to confirm your email address after registration. 
  • Agree to the terms and conditions and select Create Account.
  • Confirm your email: Click on the link in the confirmation email before it expires (within 1 hour). 

    Please note:
  • If you previous had a fitness device or app connected with your account, you will need to reconnect your device on the myHC360+ app.
  • Your account must be created DURING your program period in order to earn points.

What is my company code?

Your company code is a 5 character code, unique to your employer, used to connect you with your wellness program. The code is included in the registration instructions provided by your company or HealthCheck360.

What is my unique identifier?

Your unique identifier is usually the last four digits of your SSN or your employee ID. If you're unsure, please check the instructions provided by your company.

 

Why is providing an email address required? 

Entering your email address is required in order for myHC360+ to send you a secure link to reset your password. This provides participants with a secure platform to protect your personal health information.
