How do I complete my Health Risk Assessment Survey?

• Login to your myHC360+ account. 
• Select "My Health."
• Select "Take assessment" and then "Start HRA."
• Answer each question by selecting the answer and selecting "Next." 
• Once all questions have been answered, scroll down to review and select "Submit HRA." 

You should see "Congratulations!" once your survey is successfully submitted.
