How do I complete my Health Risk Assessment Survey?

• Login to your myHC360+ account. 
• Select Care.
• Select "Take assessment" and then Start HRA.
• Answer each question by selecting the answer and selecting Next.
• Once all questions have been answered, scroll down to review and select Submit HRA. 

You should see "Congratulations!" once your survey is successfully submitted.
