Because of privacy rights, we cannot see or reset your password, but you can reset your password by following these steps:

• On the login screen of the myCare360 mobile app or online at com, click on “Reset your password” link.

• Enter your personal information
• Company Code
• The last 4 of your social
• Click “Reset Password”
• Note that resetting your password on MyCare360 also updates your password on MyHealthCheck360
