How do I submit my HealthCheck360 Physician Wellness Screening Results Form?

Steps to submit your completed form

Online Portal

1. Log into your myHealthCheck360 account at myHealthCheck360.com.
2. Click Download/Submit Physician Form on the Submit a Physician Form tile on your dashboard.
3. Complete the submission page ensuring all required fields are thoroughly and accurately filled out according to your form.
4. Upload your completed form at the bottom (Required)
   1. If you do not have a copy of your form, please contact your physician’s office.
5. Check the consent box to release health information.
6. Click Submit 
   
   Your submission will be reviewed by the HealthCheck360 team. It will then be approved or denied accordingly and you will receive notification via email. 
   
   

Mobile App

1. Log into your myHealthCheck360 account on the myHealthCheck360 app
2. Tap on the “menu” icon in the top right corner, then “Download/Submit Physician Form”
3. Complete the submission page and upload your completed form by taking a picture of your form or select the photo from your gallery.
