How do I take my survey?

Learn how to access your health survey on the mobile app and online.

Taking Your Survey On the Mobile App

1. Log in to your account
2. Once signed in, click on My Health from the bottom navigation menu
3. Click the Take Survey button
4. Select Take Survey to launch your health survey
5. Click Continue and complete the questions on the survey

Taking Your Survey Online

1. Go to: www.myHealthCheck360.com
2. Click on the “Login” button on the home page
3. Log in to your account
4. Once signed in, click the white box that says 'Take Survey'
5. Complete the questions on the survey

After you complete your survey, the option to take the survey will disappear indicating that you have finished the survey.

You can review the date you completed your survey by navigating to My Program and clicking on Survey & Results.
