Learn how to submit Lifestyle Rewards activities on the app and online
On the myHealthCheck360 App
- At the bottom select Rewards
- Select + Add
- Select your activity from the drop down menu
- Type a brief activity description, enter a date, and upload your document if required
- Select + Add to submit your activity
On Your Computer at www.myhealthcheck360.com
- Click Lifestyle Rewards on the left hand menu
- Click the blue Submit Activity box
- Select your activity from the drop down menu
- Type a brief activity description, enter a date, and upload your document if required
- Select Submit to submit your activity

For activities that require a file to be uploaded, the limit is 10 MB. Accepted file formats include: .pdf, .jpg, .png, .gif, .doc, .docx
