How do I download a Physician Form?

Obtain a Physician Form by logging into your myHC360+ account.

  • Select "Biometric Screening" from your to-do list on your homepage.
  • Select "Manage a screening" Or on the website, find "Physician Form" under Biometric Screening.
  • Scroll down to select "Physician Form"

 

How do I schedule?

  • Contact your Healthcare Provider and schedule an annual wellness appointment. 
  • Ensure your scheduled visit is covered as an annual wellness appointment by asking your Healthcare Provider or insurance company. 
  • Ensure your scheduled visit is within the program deadline.


What should I bring to my appointment?

  • Download the Physician Screening Form from your myHC360+ account.
  • Complete the Participant section before your appointment.
  • You must sign your results form.

 

How do I submit my Physician Form?

  • Log in to your myHC360+ account. 
  • Select the Health tab at the bottom of your screen. 
  • Select Physician form under the Biometric Screening section.  
  • Select Upload PDF to upload your completed screening form and fill in the on-screen form.
    • Accepted formats include: pdf, jpg, png, gif, doc, and docx
  • Once submitted, you will receive a notification that results are under review.

Note: The Physician Form upload will require a phone number to be entered in the following format +1XXXXXXXXXX (ex. +18665110360). Enter +1 followed by your 10-digit phone number with no spaces or characters.

 

Keep in mind:

  • Employees and spouses must complete separate forms and submit them through their separate myHC360+ accounts. 
  • Your results are available within 10 business days after they are received on myHC360+. If your results are not available within 10 business days, please confirm all required fields were included on your form.
