Learn how to complete your health risk assessment.
Watch here or read below!- Login to your myHC360+ app or login to the myHC360+ website
- Select "Health Risk Assessment" from your To-Do list on the homepage or you can access your Health Risk Assessment from the Health tab on the bottom navigation bar and selecting "Take assessment"
- Select "Complete survey"
- Select "Take assessment"
- Select "Start HRA"
- Answer each question by selecting the answer and selecting "Next"
- Once you have answered each question, you will have the opportunity to review your survey answers and adjust any answers you may want to change. If you would like to change an answer select "back" in the left-hand corner.
- Once you're finished, select "Submit HRA"
You should see "Congratulations!" once your survey is successfully submitted.
