What Should I Do If I Forgot My Username or Password?

Retrieve Your Username:

1. Select "Forgot username" on myHC360+
2. Enter the email address associated with your account and select “Retrieve username.” 
   
3. Your username will be sent from support@healthcheck360.com to your registered email address.  

Reset Your Password:  

1. Select "Forgot password?"  
2. Enter your username.  
   
3. A password reset link will be sent from support@healthcheck360.com to your registered email address.  
4. Click the link and follow the prompts in the email to reset your password.
