View Your Health Report on the myHC360+ Mobile App
- Log into your myHC360+ mobile app
- Select "My Health" from the bottom navigation bar
- Tap on each biometric for more information
How do I see my past results?
- Log into your myHC360+ mobile app
- Select "My Health" from the bottom navigation bar
- Select "Past Results"
- Select the year from the drop down menu
- Select "View Results"
Want to share your results?
Sharing your results with your physician is easy! Either bring your myHC360+ mobile app with you to your appointment, or fax a summary of your results directly to your Physician from the myHC360+ app. To fax your results:
- Log into your myHC360+ account
- Select "My Health" from the bottom navigation
- Fill in your contact information and your physician’s fax information
- Select "Submit"
- Select "Fax Report to Physician" found at the bottom of the screen
- You will receive a notification when the fax has been successfully sent to your physician
