Where can I find my screening results?

Learn how to locate your screening results and share your results with your physician.

View Your Health Report on the myHC360+ Mobile App

  • Log into your myHC360+ mobile app
  • Select "Care" from the bottom navigation bar 
  • Under Wellness, view your results
  • Tap on each biometric for more information

How do I see my past results? 

  • Log into your myHC360+ mobile app
  • Select "My Health" from the bottom navigation bar
  • Your current year’s screening results will appear. To view previous results, select a different year from the drop down at the top. 

Want to share your results? Fax a summary of your results directly to your Physician from myHC360+. 

  • Log into your myHC360+ account
  • Select "Care" from the bottom navigation 
  • Fill in your contact information and your physician’s fax information
  • Select "Submit"
  • Select "Fax Results to Physician."
  • You will receive a notification when the fax has been successfully sent to your physician
