How do I retrieve my username?

You must reset your password in order to see your username.

If you forgot your username, please reset your password to retrieve it. 

1. On the login screen of the mobile app or online at www.MyHealthCheck360.com, click on “Forgot username or password” link.
2. Enter your personal information
3. Click 'Reset Password'
4. Once complete the site will remind you of your username
