The Cognizant Wellbeing Rewards Program allows associates to earn points for the completion of various preventive wellbeing activities¹. These activities are encouraged to help keep both mind and body sharp year-round while also making you eligible for the Divvy Rewards Mall²drawings done quarterly.

New in 2026! Your mission is to focus on intention this year as you work towards achieving the various program milestones:

Prizes valued at $250

Quarter 2 (Apr - June) Checkpoint:

Quarter 3 (July - Sept) Checkpoint:

The Grand Prizes valued at $1,000 will be drawn at the beginning of December and the raffle will include any associates who achieved Wellbeing Master Status (earn 10,000+ points) by November 30, 2026.

Prizes and rewards are available only to U.S.-based Cognizant associates who meet the program requirements. Associates outside the U.S. are not eligible for prize drawings or rewards.

New to the Cognizant Wellbeing Program?

Go to myHC360+ and use Company Code CTS25. Your Unique ID will vary based on your employment status:

Cognizant Associates: full associate ID (i.e.123456)

Collaborative Solutions Associates: last 4 digits of associate ID (i.e. 3456 if full ID is 0123456)

Belcan Associates:

BEG: full associate ID (i.e.123456)
BGS: last 4 digits of SSN (i.e., 1010)
WFS: last 4 digits of SSN (i.e., 1010)

Spouses/Domestic Partners: same unique ID as associate, but add a "1" to the end (i.e., using examples above, unique ID would be 1234561, 10101, or 34561)

How do I create an account?
- Scan the QR Code to download the myHC360+ Mobile App or visit the Desktop Platform.
- Download the myHC360+ mobile from the Apple App Store or Google Play Store
- Select Register
- Enter the Company Code - CTS25
- Enter your Unique ID
  - Enter your preferred email address (email addresses can only be connected to ONE myHC360+ account)
  - Cognizant Associates: full associate ID
  - Collaborative Solutions Associates: last 4 digits of associate ID
  - Cognizant Spouses/Domestic Partners: same unique ID as the associate, but add a "1" to the end
- Create a username and password that meet the requirements
  - Can be the same as your previous account if not already taken by another Cognizant member
  - Must meet the security requirements
- Check the box next to Terms and Conditions.
Once you have created your account, try to login every few days to start out to ensure your data is syncing over and you are earning your points.
What is my company code?
I’m already signed up on myHealthCheck360. Do I still need to sign up for myHC360+?
Are spouses and domestic partners eligible to participate?

Spouses/domestic are not able to participate in the Lifestyle Rewards Program (earning points for wellbeing activities) or participate in Wellbeing Challenges. However, they are able to utilize the health tracker within the platform, set personal goals, and access the Blog (recipes, webinars, etc.).

Spouses/domestic partners are also eligible to participate in the Health Plan Incentive Program (HPIP). Check out the Health Plan Incentive Program Page + FAQ for more information on this part of the program.
I'm a new associate. Am I eligible to participate?
- U.S. associates: Full-time U.S. associates have access to create an account and participate in the Cognizant Wellbeing Program upon hire. Those who participate in the wellbeing-related activities and achieve the 10,000-point goal will be eligible for prize drawings starting in Quarter 3. You are also eligible to participate in the Health Plan Incentive Program that allows you the opportunity to earn lower premium rates starting January 1 of the following year.
- Canadian associates: Full-time Canadian Associates have access to participate in the annual Cognizant Global Wellbeing challenge for your chance to compete against your colleagues. Canadian associates are not eligible to participate in either the Lifestyle Rewards Program OR the Health Plan Incentive Program.
How do I submit for Wellbeing Activities to earn points?
Login to your account on the myHC360+ mobile app or from the myHC360+ Web Platform:
1. Select the Program tab.
2. Select the Activities page and choose from the available list
  - Pro Tip - pin your preferred activities to your To Do List as a reminder to submit.
3. Type a brief activity description, enter a date, and upload your document if required.
  - Certain activities do require you to upload some form of documentation. Be sure to check out the description when choosing your activity from the list to understand what is required from you, the associate.
4. Submit your activity and see your points immediately.
For activities that require a file to be uploaded, the limit is 10 MB. Accepted file formats include: .pdf, .jpg, .png, .gif, .doc, .docx

What can I earn by participating in the Lifestyle Rewards Program?

U.S. associates who participate in the Lifestyle Rewards Program can be entered into quarterly drawings for achieving the checkpoint goals. Winners will be awarded credits to the Rewards Mall where you will have access to choose from a variety of items including electronics, workout equipment, gift cards, travel vouchers, and so much more!

Winners will be drawn at the top of each quarter starting in April with the final grand prize drawings being done at the beginning of December.

How do I know if I am selected as a winner for the Rewards Mall credit?

If you are selected as a winner, you will receive an email from CognizantWellbeing@healthcheck360.com notifying you that you have won with instructions to redeem your rewards. Winners will be drawn at the top of each quarter starting in April with the grand prizes being drawn at the beginning of December.
