Be sure to take the following steps to ensure a successful visit:
1. Bring the necessary paperwork to your appointment.
- If you are going to a LabCorp clinic, bring your Lab Requisition Form to your appointment to ensure you are not charged.
- If you are completing with your Primary Care Provider, download the Physician Form from your myHC360+ account and provide it to your PCP for review and completion. Be sure to point out to your provider that the visit should be coded as PREVENTATIVE and not diagnostic, so that you are not incurring any out-of-pocket expenses.
- Spouses and domestic partners getting screened must print and bring their own individual forms. Failure to do so may result in having to re-screen and, in some cases, out-of-pocket expenses.
2. Fast 8 - 12 hours before your screening. Do not eat or drink anything other than water 8-12 hours prior to your screening. Talk with your personal physician before fasting if you are diabetic, pregnant, or taking any prescribed medication(s).
3. Drink lots of water. Drink plenty of water the night before and the morning of your screening. Staying hydrated will help make the screening process more comfortable.
4. Arrive 5 - 10 minutes early. Registration will take a few minutes. Arriving early will keep things moving on time, so you can get back to your day as soon as possible.
5. Relax. If you are feeling nervous or have difficulty giving blood, please tell the certified health professional.
6.Know your numbers. As you prepare for your health screening, log in to your myHC360+ account to review your previous year’s individual report.
