• How do I create an account?

  

   

  • Scan the QR Code to download the myHC360+ Mobile App or visit the Desktop Platform.
  • Select Register
  • Enter the Company Code - CTS25
  • Enter your Unique ID
    
    • Cognizant Associates: full associate ID
    • Collaborative Solutions Associates: last 4 digits of associate ID
    • Belcan Associates:
      
      • BEG: full associate ID
      • CGS: last 4 digits of SSN
      • WFS: last 4 digits of SSN
        
    • Spouses/Domestic Partners: same unique ID as the associate, but add a "1" to the end

  • Enter your preferred email address (email addresses can only be connected to ONE myHC360+ account)
  • Create a username and password that meet the requirements
  • Check the box next to Terms and Conditions. 
    

  
  

  Once you have created your account, try to login every few days to start out to ensure your data is syncing over and you are earning your points. 

• What is my company code?

• I'm a new associate. Am I eligible to participate?

  • U.S. associates: Associates hired on or after August 1 will automatically receive discounted rates and will not need to participate in the incentive program until it becomes available to them in a future benefits cycle.
  • Canadian associates: The Health Plan Incentive Program and Lifestyle Rewards programs are not available to Canadian associates. However, full-time Canadian associates may create an account and access resources within the myHealthCheck360 platform (such as trackers, social challenges, and goal setting), and may also participate in Cognizant Global Wellbeing challenges.

• Are spouses and domestic partners eligible?

  Yes, spouses and domestic partners who are, or will be, covered under a Cognizant medical plan are eligible to participate in the Health Plan Incentive Program.

• What can I earn by participating in the Health Plan Incentive Program?

  You (and your spouse/domestic partner) can participate in a biometric health screening and nicotine attestation to receive a discount on your medical contribution of $50 a month per user, per completed activity.  That’s a total annual discount of up to $2,400.

• How do I complete a biometric screening?

• How do I prepare for my biometric screening?

  1. Bring the necessary paperwork to your appointment.

  • • If you are going to a LabCorp clinic, bring your Lab Requisition Form to your appointment to ensure you are not charged. You must register via myHC360+ and bring the 2026 Lab Requisition Form to ensure results are processed successfully.

    • If you are completing with your Primary Care Provider, download the Physician Form from your myHC360+ account and provide it to your PCP for review and completion. Be sure to point out to your provider that the visit should be coded as PREVENTATIVE and not diagnostic, so that you are not incurring any out-of-pocket expenses.

    • Spouses and domestic partners getting screened must print and bring their own individual forms. Failure to do so may result in having to re-screen and, in some cases, out-of-pocket expenses.

  2. Fast 8 - 12 hours before your screening. Do not eat or drink anything other than water 8-12 hours prior to your screening. Talk with your personal physician before fasting if you are diabetic, pregnant, or taking any prescribed medication(s).

  3. Drink lots of water. Drink plenty of water the night before and the morning of your screening. Staying hydrated will help make the screening process more comfortable.

  4. Arrive 5 - 10 minutes early. Registration will take a few minutes. Arriving early will keep things moving on time, so you can get back to your day as soon as possible.

  5. Relax. If you are feeling nervous or have difficulty giving blood, please tell the certified health professional.

  6.Know your numbers. As you prepare for your health screening, log in to your myHC360+ account to review your previous year’s individual report.

• Where can I view my current and past health reports and health scores?

  Your individual health results can be found in your myHC360+ account under the Care tab. To toggle through your past results, find the Select Year dropdown menu found above "My Score" and select the preferred year.

  Under your health score, you have the ability to share your individual health results with your Primary Care Physician via fax or download the PDF to save in your records.

  The health information you share with HealthCheck360 is kept completely private and secure. It will never be shared with your employer or used for anything other than helping you understand your health and providing tools to maintain and improve it.
   

   

• Why don’t I see my health report?

  Has it been 10 business days since your LabCorp appointment or since you uploaded your Physician Form? If not, please give it until the end of this window. If it has been at least 10 business days, please contact HealthCheck360 at 833-216-4558 or CognizantWellbeing@HealthCheck360.com.

    💬"Hi my name is [your name] with Cognizant. I completed a screening on [DATE] through [LabCorp/my Physician] and I do not see my results."

• What resources are available to improve my health score?

  HealthCheck360 offers optional coaching programs to associates enrolled in a Cognizant Medical Plan to help you meet your health goals, including:

  • A 12-week Pre-Hypertension Coaching Program for elevated blood pressure 
  • A 12-week Pre-Diabetes Coaching Program for elevated blood sugar and/or A1c

• How do I complete the Nicotine Attestation?

• How do I change the response of my nicotine response?

  If you have submitted an incorrect response on your Nicotine Attestation, contact HealthCheck360 Support at 833-216-4558.or CognizantWellbeing@healthcheck360.com. You will not see these changes reflected within your account, but HealthCheck360 will ensure your correct response is reflected in your incentive.

• What resources are available to aide in quitting nicotine/tobacco use?

  HealthCheck360 12-week Nicotine Cessation Coaching Program (Available to associates enrolled in a Cognizant Medical Plan Only)
  

  To enroll in this program, select the following response on your Nicotine Attestation:

  ⏺️ "Yes, I attest to being a nicotine user, please enroll me in the 12-week Nicotine Cessation Program."

• Will my information be confidential?

  Yes. HealthCheck360 follows the same confidentiality standards as your doctor. Protected health information at an individual level will not be shared with any other parties, including Cognizant. To review our Privacy Policy, please click here.

• I currently reside outside the U.S. but have transferred/will be transferring into the U.S. Do I still need to participate to earn the incentive?

  If your transfer to the U.S. occurs on or after August 1, you will automatically receive discounted rates for the upcoming plan year and will not need to participate in the incentive program until it becomes available to you in a future benefits cycle.

  If your transfer to the U.S. occurs prior to August 1, you will be expected to participate in the program to receive discounted rates for the upcoming benefits cycle.
