• How do I create an account?

  

   

  • Scan the QR Code to download the myHC360+ Mobile App or visit the Desktop Platform.
  • Download the myHC360+ mobile from the Apple App Store or Google Play Store
  • Select Register
  • Enter the Company Code - CTS25
  • Enter your Unique ID
    • Enter your preferred email address (email addresses can only be connected to ONE myHC360+ account)
    • Cognizant Associates: full associate ID
    • Collaborative Solutions Associates: last 4 digits of associate ID
    • Cognizant Spouses/Domestic Partners: same unique ID as the associate, but add a "1" to the end

  • Create a username and password that meet the requirements
    • Can be the same as your previous account if not already taken by another Cognizant member
    • Must meet the security requirements
  • Check the box next to Terms and Conditions. 
    

  
  

  Once you have created your account, try to login every few days to start out to ensure your data is syncing over and you are earning your points. 

• What is my company code?

  CTS25

• I’m already signed up on myHealthCheck360. Do I still need to sign up for myHC360+?

  As of April 1, 2025, the myHealthCheck360 platform is no longer available.  All members must sign up for a new account on the new myHC360+ app.

• What should I do with the old myHealthCheck360 mobile app?

  As of April 1, 2025, access to the myHealthCheck360 platform will be turned off. You can delete the myHealthCheck360 mobile app from your devices.

• I'm a new associate. Can I still participate?

  • U.S. associates: Full-time U.S. associates have access to create an account and participate in the Health Plan Incentive Program to be eligible for lower premium rates starting on January 1 (Medical Plan renewal). Those hired on or after 08/01 of the current program year will receive the discounted rates (including spouse/domestic partner) and will not be required to participate in the requirements for that program year. However, you will be expected to participate the following year and years after to keep the discounted rates.
  • Canadian associates: The Health Plan Incentive Program and Lifestyle Rewards programs are not available for Canadian associates. Full-time Canadian associates may, however, create an account and utilize the resources within the myHealthCheck360 platform (trackers, challenge your friends, goal setting, etc.), and also participate in the annual Cognizant Global Wellbeing challenges.

• I currently reside outside the U.S. but have transferred/will be transferring into the U.S. Do I still need to participate to earn the incentive?

  If your transfer to the U.S. is after 8/1/24, you will automatically receive the discounted rates for 2025 and will not need to participate in the incentive program until it opens in 2025 for the 2026 benefit year. If your transfer to the U.S. was prior to 8/1/24, you will have had to have participated in the program to receive the discounted rates for 2025.

• Are spouses and domestic partners eligible to participate?

  Yes, spouses and domestic partners who are, or will be, covered under a Cognizant medical plan are eligible to participate in the Health Plan Incentive Program.

• What can I earn by participating in the Health Plan Incentive Program?

  You (and your spouse/domestic partner) can participate in a biometric health screening + nicotine attestation to receive a discount on your medical contribution of $50 a month per member, per completed activity.  That’s a total annual discount of up to $2,400.
  The Health Plan Incentive Program impacting 2026 Rates will be available later this summer. Keep an eye out for more information in the coming months!
  

• Will my information be confidential?

  Yes. HealthCheck360 follows the same confidentiality standards as your doctor. Protected health information at an individual level will not be shared with any other parties, including Cognizant. To review our Privacy Policy, please click here.
