Cognizant Wellbeing Program 

Cognizant partners with HealthCheck360 to bring you a wellbeing program focused on driving awareness and creating healthy lifestyle habits. There are two components to the program:

Lifestyle Rewards Program: Complete wellbeing related activities for points.  Achieve the 10,000-point goal to be entered into prize drawings for Rewards Mall Credits. Available to all full-time U.S. associates.

Health Plan Incentive Program: Complete a Biometric Health Screening and Nicotine Attestation to be eligible for discounted medical premiums. Available to full-time U.S. associates and their spouses/domestic partners.

As of April 1, 2025, the old myHealthCheck360 platform is no longer available. All members must create a new account on myHC360+. To create an account, select Register from the login screen and enter the company code (CTS25) to be directed to set up your account using your unique identifiers. 

Cognizant Associates: full associate ID (i.e.123456)

Collaborative Solutions Associates: last 4 digits of associate ID (i.e. 3456 if full ID is 0123456)

Cognizant Spouses/Domestic Partners: same unique ID as the associate, but add a "1" to the end (i.e., using examples above, unique ID would be 1234561 or 34561)

Preferred Email Addresscan only be connected to ONE account
